ELEMENTARY NOMINATION FORM
This student is the one who works so very hard, the one who give of self without personal expectations, the one who is careful with criticisms and liberal praise, the one who promotes the principles of good government and good citizenship. Other qualities include being ambitious and willing to serve their school, their family, and their community.
Please nominate one 5th grade Girl and one 5th grade Boy for this award. The recipients will receive a plaque and a $25 Gift Card at your “End of the Year” Assembly in June.
